Offer valid only for legal residents of the 50 United States, the District of Columbia and Puerto Rico who are 18 years or older and are individual consumers.

Requests must be submitted or postmarked by October 31, 2023. Not valid with other offers. Void where taxed, restricted or prohibited. PO boxes will not be paid, except in North Dakota. Allow six to eight weeks after mailing for delivery of your Mastercard Prepaid Card**. Limit one offer per envelope and two offers per name/household/address.
